Cremation Authorization Instructions
For your convenience, we are including these authorization forms for you.
Please print out all 3 pages and write in only the name of the deceased at the top of the 1st & 2nd Page and then complete the top of the 3rd page. These instructions will print out on the bottom of the 3rd Page. Your signature will need to be witnessed but does not need to be notarized. If you have access to a fax machine please fax all 3 pages to 352-357-4140. You will need to mail all 3 pages to Harden/Pauli Funeral Home, Post Office Box 657, Eustis, Florida 32727-0657.
